Provide therapeutic social work services as a Home Health Clinical Social Worker with Fraser Health, supporting clients with significant psychosocial problems.

Reporting to a Team Leader, you will work within a community-based interdisciplinary care team. The position includes psychosocial assessments, treatment planning, consultation, family counseling, referrals, and client advocacy. You will also support caregivers and healthcare professionals.

Key Responsibilities:

- Conduct psychosocial assessments and recommend treatment plans
- Provide therapeutic interventions, family counseling, and home visits
- Consult with clients, caregivers, teams, and community agencies
- Facilitate referrals, resource access, and client advocacy




- Participate in education, research, and quality improvement

Requirements:

- Bachelor's Degree of Social Work from an accredited school
- Three years' recent related community client experience
- Current full registration with the BC College of Social Workers
- Valid BC Driver's license and personal vehicle access
- Ability to conduct assessments and communicate effectively
