This Nurse Practitioner supports older adults at the Markham Stouffville Hospital Reactivation Care Center. Working within an interprofessional team, the NP provides comprehensive nursing services, promotes best practices, and helps reactivate patients designated Alternate Level of Care. The position emphasizes evidence-based, patient-centered care.

Key Responsibilities:

- Provide comprehensive nursing services for older adults
- Assess, diagnose, and manage complex geriatric conditions
- Order and interpret diagnostic tests within scope
- Prescribe pharmaceuticals and perform authorized procedures

Requirements:

- Master’s degree and current Ontario RN(EC) registration
- Primary Health Care or Adult Nurse Practitioner designation
- Five years’ current expertise in geriatric care management
- Advanced clinical knowledge and evidence-based practice skills
- Excellent communication, leadership, and problem-solving abilities
