Extendicare is seeking a self-directed Nurse Practitioner to join our leadership team. In this full-time role, you will provide innovative clinical leadership while collaborating with physicians and staff. Your focus will be on delivering comprehensive health services to long-term care residents with complex needs, ensuring quality care through effective communication and critical thinking.
Key Responsibilities:
• Deliver primary health care for acute and chronic conditions
• Conduct comprehensive health assessments of residents
• Provide clinical leadership for the care team
• Engage with residents and families in care planning
• Promote safe resident care and mitigate risks
Requirements:
• Registration with the College of Nurses of Ontario in Extended Class
• Baccalaureate in Nursing (BScN)
• Strong communication and leadership skills
• Experience in geriatric care preferred
• Availability for flexible hours, including evenings
