Deliver patient-centered care as a Pharmacist through medication reviews, disease state management, and compassionate clinical services. This is a 12-month term position.

You will support patients, caregivers, and pharmacy colleagues through counseling, education, care plans, and documentation. The work includes injections, vaccines, adherence support, and health risk assessments. You will also help provide consistent pharmacy experiences across Fusion locations.

Key Responsibilities:

- Provide compassionate patient-centered care and customer service
- Conduct medication reviews, assessments, injections, and vaccines
- Counsel patients and caregivers about new therapies
- Initiate care plans, follow up, and document properly
- Prioritize prescription safety and delegate technical functions

Requirements:

- Bachelor of Science in Pharmacy from a recognized institution
- Licensed Pharmacist in valuable standing provincially
- Injection and First Aid Certification
- Kroll software experience preferred
- Current knowledge of pharmacy practice and regulations
