Provide advanced geriatric care at Oak Valley Health’s Reactivation Care Centre, using clinical judgment and comprehensive nursing expertise. Monday to Friday, 0900 to 1700.

The Nurse Practitioner supports older adults at the Markham Stouffville Hospital Reactivation Care Centre. Working within an interprofessional team, this position addresses health promotion, prevention, treatment, rehabilitation, and support. The NP helps reactivate older adults designated as Alternate Level of Care through direct care and consultation.

Key Responsibilities:

- Provide comprehensive nursing services for older adults
- Promote best practices in geriatric care
- Deliver direct patient care and consultation
- Support reactivation of Alternate Level of Care older adults




- Collaborate with Markham Stouffville Hospital team members

Requirements:

- Master’s Degree and current Ontario Nurse Practitioner registration
- At least five years’ current geriatric care expertise
- Advanced clinical knowledge and evidence-based practice implementation
- Competence in assessment, diagnosis, treatment, and older-adult prevention
- Solid decision-making, communication, leadership, and problem-solving skills
