Key Responsibilities:

- Conduct initial assessments and provide supportive counselling
- Coordinate services using a harm reduction approach
- Deliver chronic disease management programming

Requirements:

- Minimum two years’ recent healthcare setting experience
- Post-secondary degree in an eligible health discipline
- Maintain registration with the relevant regulatory body
- Hold a valid driver’s licence and vehicle
- Demonstrate leadership, communication, and problem-solving skills
