Based at WRHA Community Health Services - Access Downtown, this clinical position serves individuals and groups through community health services. The work includes case management, supportive counselling, service coordination, and collaboration across multidisciplinary teams. Mental health, addictions, harm reduction, and complex housing experience are relevant.
Key Responsibilities: Conduct initial assessments and provide supportive counselling
Coordinate services using a harm reduction approach
Deliver chronic disease management programming
Provide self-management programming for individuals and groups




Build relationships across agencies and community sectors
Requirements: Minimum two years’ recent healthcare setting experience
Post-secondary degree in an eligible health discipline
Maintain registration with the relevant regulatory body
Hold a valid driver’s licence and vehicle
Demonstrate leadership, communication, and problem-solving skills
Support equitable community care through case management, wellness programming, service coordination, and cooperative clinical practice.
