Key Responsibilities: Coordinate patient discharges with families, teams, and community resources
Identify needs and barriers affecting timely discharge
Implement plans supporting continuity of care
Maintain communication between hospital and community providers
Evaluate care-plan effectiveness through follow-up
Requirements: Bachelor’s degree in a related health discipline
At least two years of related skilled experience
Current registration with the appropriate regulatory College
Competency assessing pediatric health care needs
French and English bilingualism, validated through assessment
