Reporting to the Director of Care, the Licensed Practical Nurse supports residents through person-centered, goal-directed nursing care. The role includes assessments, care plans, medication administration, resident education, and interdisciplinary coordination. It also involves responding to changing conditions while upholding provincial standards, ethics, and trust.
Key Responsibilities: Conduct resident assessments and maintain accurate care documentation
Implement and evaluate care plans as required
Administer medications and treatments within scope
Coordinate care with physicians, teams,



and service providers
Respond to medical emergencies and significant condition changes
Requirements: Current provincial or state Licensed Practical Nurse registration
Current CPR and First Aid certification
Strong clinical knowledge and care coordination skills
Excellent communication, critical thinking, and time management
Electronic documentation and Microsoft Office proficiency
