The Transition Graduate Nurse provides comprehensive care across the lifespan at La Crete Community Hlth Centre. Working with a Clinical Guide, you develop independence while practicing assessment, interdisciplinary coordination, and evidence-informed care. The program serves Advanced Ambulatory Care and may include diverse healthcare settings.
Key Responsibilities: Conduct holistic assessments across the lifespan
Develop and implement individualized care plans
Coordinate care with interdisciplinary teams
Provide safe, ethical, evidence-informed nursing practice




Contribute to health system improvement
Requirements: Graduate from an accredited nursing education program
Active or eligible CRNA registration and practice permit
Current Basic Cardiac Life Support Health Care Provider certification
Graduate within twelve months before program start
Ability to triage, delegate, communicate, and provide patient care
Build independent registered nursing practice through clinical guidance, compassionate communication, and comprehensive care in northern Alberta.
