Positive Shepherd Centres in Toronto is seeking an Intensive Case Manager to support clients facing mental health and substance use challenges within our Non-Profit Homes. You will assess needs, develop person-centered goals, and coordinate services to promote recovery and independence.
With a background in social work or related fields and an addictions certificate, you will manage a caseload, liaise with community resources, and advocate for clients in a respectful, anti-racist framework.
