Provide intensive case management and advocacy with YSSN in Aurora, supporting people with developmental disabilities across York Region.
As a Developmental Services Case Manager, you will support individuals aged six and older with developmental disabilities or dual diagnosis. Using person-centered service delivery, you will help clients and families navigate developmental services, access community resources, and explore support options. The role combines assessment, planning, referrals, and skill development to support independence and community participation.
Key Responsibilities: Provide intensive case management and advocacy services
Conduct needs assessments for individuals and families
Develop and coordinate individualized service plans




Facilitate referrals to appropriate community resources
Support skill development, independence, and community participation
Requirements: Bachelor’s degree in social work or related discipline
Minimum three years of directly related frontline experience
Experience working with developmental disabilities
Crisis prevention, intervention, and sound clinical judgment
Valid G license and reliable transportation, or alternative transportation
Bring case management, advocacy, assessment, and communication skills to help YSSN clients achieve meaningful goals across York Region.
