Support dementia and responsive behaviours as a Registered Practical Nurse with Lakeridge Health in Oshawa, providing home-based assessment and intervention.
This temporary full time, 18-month position serves the Behavioural Supports Ontario Program and GAIN team. The clinician provides bio-psychosocial assessment, behavioural care planning, care coaching, education, and modelling. Collaboration includes hospital and GAIN Community teams, GEM nurses, CCAC, and other practitioners.
Key Responsibilities: Provide direct support to clients in their homes
Assess dementia, mental health, addictions, and neurological concerns
Lead person-centred behavioural care planning and intervention
Coach caregivers and provide education through behavioural modelling




Liaise with teams and practitioners across the circle of care
Requirements: Current Ontario RPN registration in good standing
Current BCLS certificate and full RPN scope
Five years supporting community clients and conducting home visits
PIECES, Montessori Based Dementia Care, and GPA certificates
Valid driver licence and personal transportation for required travel
Bring gerontological assessment, dementia expertise, and person-centred behavioural care to Lakeridge Health clients at home.
