Job Overview

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The RPN will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. Duties

- Deliver direct patient care in accordance with physician orders and established nursing protocols
- Administer medications and treatments safely and accurately, including injections and phlebotomy procedures
- Monitor patient vital signs and assess health status regularly
- Assist with activities of daily living and provide emotional support to patients and their families
- Document patient information accurately using electronic health records systems such as Epic or Cerner
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Manage case documentation for patients requiring complex or chronic care management
- Support specialized care needs including dementia, pediatrics, hospice, pain management,



and senior care
- Educate patients and families on health conditions, medication use, and post-discharge care instructions
- Maintain a secure environment by adhering to infection control protocols and safety guidelines

Requirements

- Valid registration as a Practical Nurse (RPN) with the appropriate licensing body
- Proven experience in nursing home, hospital, acute care, or assisted living settings is highly preferred
- Experience working with diverse populations such as pediatric patients, seniors, or individuals requiring dementia or hospice care
- Strong knowledge of medical terminology, medication administration, and clinical procedures such as phlebotomy and laboratory testing
- Familiarity with electronic health record systems like Epic or Cerner is advantageous
- Excellent communication skills with the ability to collaborate effectively within multidisciplinary teams
- Ability to handle multiple priorities while maintaining attention to detail in fast-paced environments
- Compassionate demeanor with a commitment to delivering patient-centered care
