Extendicare is seeking a self-directed Nurse Practitioner to join our leadership team. In this full time role, you will provide innovative clinical leadership while collaborating with physicians and staff. Your focus will be on delivering comprehensive health services to long-term care residents with complex needs, ensuring quality care through effective communication and critical thinking.
Key Responsibilities:
• Deliver primary health care for acute and chronic conditions
• Conduct comprehensive health assessments of residents
• Provide clinical leadership for the care team
• Engage with residents and families in care planning
• Promote safe resident care and mitigate risks
Requirements:
• Registration with the College of Nurses of Ontario in Extended Class
• Baccalaureate in Nursing (BScN)
• Strong communication and leadership skills
• Experience in geriatric care preferred
• Availability for flexible hours, including evenings
