Winnipeg Regional Health Authority in Winnipeg is seeking a term Community Health Services Case Coordinator to support Home Care River Heights/Fort Garry. This in-person, 1.00 FTE role operates within WRHA Community Health Services
- Access Fort Garry, with anticipated start and end dates from 11/16/2026 to 05/15/2028. Candidates should hold a health/human service degree (BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD or related) and have two years of related community experience;
