Registered Nurse or Registered Practical Nurse Care Supervisor needed in Toronto, coordinating assessments, care plans, documentation, and patient safety. This is a one-year contract.

CBI Home Health seeks a temporary Toronto Care Supervisor for its community service delivery. The position requires current College of Nurses of Ontario registration, supervisory or home health experience, and reliable transportation. You will support patients at home and in the community through timely referrals, reassessments, risk management, and staff guidance.

Key Responsibilities:

- Respond to internal and external patient referrals within timelines
- Complete comprehensive assessments using established tools and procedures
- Develop care plans addressing individual patient needs
- Document client status,



care planning, and charting accurately
- Investigate complaints and respond to incident reports

Requirements:

- Current CNO registration as Registered Nurse or Registered Practical Nurse
- Two years supervisory or home health experience
- Valid Ontario driver’s license and reliable vehicle
- Ability to build rapport with clients and Care Staff
- Ability to work independently and collaboratively
