You will collaborate with an interprofessional team to manage client caseloads, provide direct care, oversee care plans, and support transitions to primary/community care.

The role emphasizes teaching, self-management, and wellness, with a focus on preventing avoidable hospital admissions and improving quality of life for clients living at home.
