WNHAC in Ontario is seeking a Primary Care Nurse Care Coordinator to join a client-centered, inter-qualified team. The role includes nursing assessments, immunizations, wound care, and medication administration within the nursing scope of practice.

You will collaborate with Home and Community Care staff and Community Health Nurses to support care measures, maintain medical records, participate in quality improvement, and travel to community sites as required.
