NorWest Community Health Centres in Thunder Bay, ON, seeks a full-time Intake Registered Nurse to coordinate client attachment into primary care sites and programs, using clinical assessment and navigation to connect clients to services.

You will collaborate with physicians, nurses, and allied health professionals to ensure timely access, manage referrals, and uphold health equity; HOOPP and excellent perks are provided.
