Key Responsibilities:
• Assess and enhance patient pharmacotherapy outcomes
• Ensure safe drug distribution and procurement management
• Mentor new hires and support continuous education
• Engage in development and review of pharmacy initiatives
• Liaise with patients and healthcare teams for effective care
Requirements:
• Graduation from an accredited Pharmacy program required
• Registration with the Ontario College of Pharmacists is necessary
• Hospital pharmacy experience preferred but not mandatory
• Outstanding verbal and written communication required
• Knowledge of rehab clinical practices is an advantage
