Manage daily clinical priorities, including referrals, weight monitoring, RAI assessments, and new Resident admissions

participate in morning huddles where applicable

Review Resident charts and data (weights,



intake, labs, medications, diagnoses, and health changes) to assess nutrition risk

Spend time in the dining room observing meals, engaging with Residents, and supporting consistent nutrition care with staff

Occasionally support menu approvals, quality data collection, and organizational initiatives

This role provides a balanced blend of independent clinical practice, teamwork, and on‑site engagement across Homes.

A people-first approach with strong customer-service skills and a commitment to building respectful, trusting relationships

Proficiency with EMR systems and digital documentation

Must provide proof of liability insurance, TB testing and vulnerable sector check

Travel may be required for certain positions
