Key Responsibilities: Process funder referrals and updates in Procura
Assess service needs with supervisors, nurses, and patients
Assign care providers using needs, geography, skills, and availability




Notify patients and staff about schedules
Resolve patient concerns through escalation pathways
Requirements: Secondary School Diploma
Minimum two years of related customer service experience
Working knowledge of medical terminology and records
Excellent Windows, Microsoft Office, internet, and EMR skills
