The North Simcoe Family Health Team (NSFHT) is seeking a full time, permanent position.

Job Title: Mental Health Clinician - Social Worker or Psychotherapist

Salary range: $38.48 - $40.83 per hour

Reports to: Clinical Director

Position Summary

The Mental Health Clinician provides high-quality, brief, strength-based counselling to patients in a primary care setting. This role focuses on psychosocial assessments, counselling, and system navigation support, including connecting patients and families with appropriate community and social resources. This position does not involve case management or crisis intervention and operates within a scheduled, short-term treatment model.

Responsibilities

- Deliver short-term, brief strength-based counselling (e.g., Solution-Focused Therapy, CBT, DBT-informed skills).
- Conduct focused evidence-informed psychosocial assessments to identify patient strengths and wellness goals.
- Collaborate with the interdisciplinary team and Clinical Coordinator to support patient wellness through short-term clinical interventions.
- Support patients with housing, financial, family, and social determinants of health concerns.
- Support patients in developing self-management strategies for mild to moderate mental health concerns.
- Maintain accurate clinical documentation in the EMR according to professional standards.

Qualifications

- Master of Social Work (MSW) preferred; Bachelor of Social Work (BSW) with experience will be considered.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or College of Registered Psychotherapists of Ontario (CRPO).
- Minimum of 2 years of experience in clinical counselling, with expertise in brief, solution-focused models.
- Proven ability to work effectively within an interprofessional primary care team.

Work Schedule:

This is a full-time, permanent position with a standard 37.5-hour work week, Monday to Friday.
