Personal Support Worker (PSW) – Community & Home CareJob Overview

City Healthcare Services Inc. is seeking compassionate, reliable, and dedicated Personal Support Workers (PSWs) to provide quality care to clients in their homes within the community.

We are hiring PSWs for both community-based home-to-home visits and one-on-one care assignments at a single client location, depending on client needs, worker availability, and scheduling preferences.

The ideal candidate is passionate about helping seniors, individuals with disabilities, and clients requiring personal assistance maintain their independence, dignity, and quality of life in the comfort of their own homes.

Work Settings:

- Community Home Care: Travel between multiple clients' homes within an assigned geographical area.
- One-on-One Home Care: Provide dedicated care to an individual client at a single location during scheduled shifts.
- Full-time, part-time, and casual opportunities may be available.
- Flexible scheduling, including weekdays, evenings, weekends, and holidays.

Key Responsibilities

- Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene.
- Provide mobility assistance, protected transfers, repositioning, and support with mobility equipment.
- Assist with medication reminders and medication-related tasks as authorized by the care plan, training, and applicable policies.
- Prepare meals and assist with feeding according to clients' dietary requirements.
- Perform light housekeeping duties, including laundry, tidying,



and maintaining a safe home environment.
- Provide companionship, emotional support, and meaningful social interaction.
- Support clients with dementia, Alzheimer's disease, physical disabilities, and other care needs.
- Observe and report changes in clients' physical, emotional, or cognitive conditions.
- Follow individualized care plans and accurately document care provided.
- Travel between clients' homes for community assignments or provide continuous one-on-one support at a designated location.
- Maintain professional communication with clients, families, supervisors, and healthcare professionals.

Qualifications & Requirements

- Personal Support Worker (PSW) certificate from a recognized educational institution.
- Previous experience in community home care, private home care, retirement homes, or long-term care is an asset.
- Experience supporting seniors, individuals with disabilities, and clients with dementia is preferred.
- Valid CPR and First Aid certification.
- Valid Vulnerable Sector Check or willingness to obtain one.
- Knowledge of infection prevention and control practices.
- Strong communication, time management, and organizational skills.
- Ability to work independently in clients' homes.
- Reliable,



compassionate, punctual, and committed to providing quality care.
- Ability to maintain confidentiality and respect clients' dignity and privacy.
- Valid driver's licence and access to a reliable vehicle are preferred for community home-to-home assignments.
- Ability to travel within assigned service areas when required.
