POSITION SUMMARY :

Reporting to the Director, Clinical Services, and in cooperation with the patient/family and other professionals, the Pharmacist designs, implements and monitors medication therapy specific to the patients' needs. The Pharmacist identifies, resolves and prevents medication related problems and contributes to positive patient outcomes. The Pharmacist works with and supports the centralized dispensary computerized drug distribution system to ensure medications reach the patient in a secure and timely manner. Allduties are performed in accordance withthe Mission andValues of Providence Healthcare.

MAJOR RESPONSIBILITIES:

- Complete medication reconciliation upon admission,
