Halton Healthcare is seeking a Registered Nurse for a Complex Continuing Care unit. You will provide specialized nursing care to medically complex patients and support families throughout the care transition.

You will coordinate care plans, educate families, and collaborate with the health team to improve patient outcomes. The RN will administer medications, perform procedures per policy, and participate in discharge planning.
