Our RAI/MDS Coordinators manage the process of coding and utilizing the MDS assessment system (including RAPs, QIs, RUGs and Outcome Measures) to manage risk, optimize funding, identify Quality of Care issues, and measure performance. They provide education and guidance to staff and management and advise on training needs, coding issues, organizational scoring and emerging trends related to RAI/MDS matters. The RAI/MDS Coordinators participate as part of our dynamic inter-professional team and are invaluable in ensuring recognition of the essential elements of care that can improve the quality of life for our residents.

Additional duties include:

· Ensuring comprehensive assessments of resident care needs in the areas of psychosocial status, communication, activity levels, cognition and physical condition;





· Liaising with the Canadian Institute for Health Information (CIHI), CCRS and the Ministry of Long Term Care regarding reporting and interpretation/dissemination of information and reports received from CIHI;

· Monitoring quality indicators as they relate to the RAI/MDS coding processes; and

· Monitoring the integrity of data collection, entry and submissions as required by the Canadian Institute for Health Information (CIHI).

Qualifications:

· Registered Practical Nurse diploma

· Current registration with the College of Nurses of Ontario

· Experience with MDS, care plans and related documentation

· Familiarity with long term care, the Long Term Care Homes Act and Regulations, and MOLTC Compliance

· Robust project management, organizational and communications skills

· Experience and confidence in providing education to adult learners

· Completion of MDS Credentialing Course is a definite asset

Pay: $30.25-$37.81 per hour
