Extendicare Countryside is seeking an in-home behavioral support lead to focus on residents, families and staff. The role emphasizes case management, behavioral assessments, and collaboration with an interdisciplinary team to implement effective care plans.

The incumbent will liaise with community resources, chair the Responsive Behavior Committee, and contribute to care conferences. Compensation details indicate an hourly range of CAD 33.47–CAD 35.07 for a 75-hour bi-weekly schedule.
