- CR06) ? What You’ll Do:
• Daily supporting clients to work towards their Individual Support Plan
- client goals and identified measurable outcomes.
• Performing a thorough and complete shift exchange with previous shift.
• Using Catalyst system while administering medications to clients on time and checking/receiving medication that arrive while on shift.
• Maintaining a clean, secure, and comfortable environment for clients.
• Making Guardians feel welcome into the client’s residences and communicating/interacting with guardians of clients as directed by the site supervisor.
• Supporting client’s care needs such as: hygiene, dressing, making beds, toileting, etc.
• Assisting and monitoring clients through the entire shift.
• Completing, reading, and maintaining all required documentation.
• Planning for, preparing, and documenting meals and snacks for clients.
• Performing other administrative duties such as weekly confirmations, order money, end of the month client service updates, send in occupancy reports, and any other assigned tasks.




• Performing daily and weekly household duties such as: dusting, washing walls, vacuuming, cleaning, laundry, washing windows, shovelling snow, cleaning bathrooms, washing dishes, etc. ensuring a safe and comfortable environment for clients. ✅ What You Bring:
• A caring, reliable, and person-centered approach.
• Experience with addictions, harm reduction and extreme trauma.
• Strong communication skills
- ASL proficiency is highly preferred
• Valid First Aid-CPR Level C Certificate is a mandate.
• Valid police information check with vulnerable sector is mandate.
• Willingness to follow established policies, procedures, and support plans
• Basic computer skills are required
