Position: Senior Project Manager, Epic Connect THP/HH Status: Temporary Full-Time (24 months) Number of Vacancies: 1 Department: Epic Connect Role Level: $48.85-$61.06 (salary band placement commensurate with experience) Reports To: Associate Vice-President, Epic Connect THP/HH Posted: Tuesday, October 20, 2026 – Tuesday, November 3, 2026 Strategic Context Trillium Health Partners (THP) and Halton Healthcare (HH) are formally exploring whether HH should join THP’s Epic environment through Epic Connect. The due diligence and planning phase is focused on the assessment and planning to validate the chance, confirm requirements, and determine the recommended direction. It does not represent a final implementation decision or commitment. Together, HH and THP aim to create a more connected, accessible and digitally enabled regional system of care that improves patient outcomes, strengthens team and learner experience, and makes the best use of public resources for the communities they serve. The potential partnership is a clinical transformation enabled by digital, operational, and governance alignment. The due diligence and planning phase will assess clinical workflow and THP foundation fit, technical and infrastructure requirements, scope, total cost of ownership, resourcing, governance, organizational readiness, stakeholder engagement and the future operating model. Findings will inform a proposed Epic Collaboration Agreement and implementation recommendation for approval by the appropriate governance bodies. The Position Reporting into the Associate Vice President, Epic Connect THP/HH and working closely with the Project Director, Epic Connect, the Senior Project Manager is a critical leadership role and the central program integrator for the due diligence and planning phase. The success of this complex, cross-organizational initiative will depend on this role’s ability to establish, maintain and track or document one reliable view of scope, milestones, interdependencies, resources, risks, decisions and outcomes across both organizations. By connecting strategy, governance and execution, the Senior Project Manager will provide discipline, foresight and evidence leaders need to make timely, informed decisions and keep all workstreams aligned. This is not a scheduling role; it is a strategic delivery, capability-building and executive decision-support role that is essential to reducing program risk, maintaining momentum and ensuring the partnership is positioned to mobilize successfully into a future phase, if approved. The role will also establish repeatable program disciplines that can support future regional partnerships. If the organizations proceed following due diligence, the role will transition into the implementation team, supporting detailed planning, clinical workflow alignment, organizational readiness, implementation execution, go-live, and post-go-live stabilization while maintaining continuity of decisions, risks and interdependencies. This role will help ensure a seamless transition from planning into implementation and adoption across THP and Halton Healthcare. Key Accountabilities
• Build and maintain the integrated due diligence and planning phase roadmap across launch, tracking clinical and technical scope validation,

financial validation, project and operating governance, resource and operating model, and Epic Collaboration Agreement development.
• Establish integrated controls for scope, schedule, budget, resources, risks, issues, dependencies, changes, benefits, and readiness.
• Maintain the cross-organizational roles and responsibility matrix or can use governance and ownership matrix and governance calendar, including agendas, pre-reads, records of decision and action follow-through.
• Prepare concise executive reporting that summarizes findings, options and implications, recommendations and identifying decisions required – while distinguishing the current planning phase from any future implementation.
• Coordinate clinical, technical, infrastructure, application, finance, privacy, security, legal, communications, stakeholder engagement, organizational-readiness and operating-model workstreams.
• Support consensus decision-making at the Joint Steering Committee and ensure unresolved material issues are escalated through the approved pathway.
• Coordinate with Ontario Health, the Board(s), Epic, supplier and other approval or consent of dependencies reflected in the program plan.
• Integrate total cost of ownership, resourcing, operating implications and technology-capacity requirements into the overall recommendation.
• Ensure program communications consistently explain the purpose of the due diligence and planning phase, what is being assessed, what has been decided, what remains to be confirmed, and how clinicians and teams will be engaged.
• Protect confidential information and ensure program documentation supports the Parties agreed on confidentiality, cost-recovery and evidence requirements. What Success Looks Like
• Leaders have an accurate integrated view of progress, risk, resources, decisions, and readiness.
• Due diligence and planning deliverables are filtered through the approved work program and decision points.
• Decisions are documented, evidence-based, and risks are escalated through the approved governance model.
• The proposed agreement and implementation recommendation are supported by traceable clinical, technical, financial, and governance evidence.
• The program can mobilize into the implementation phase without losing strategic intent or decision history, and THP retains reusable methods and tools for future Epic Connect and digital partnership opportunities. Qualifications and Leadership Profile
• An undergraduate degree in health administration, business, information systems, engineering, a clinical discipline or related field; is required.
• At least 5 years of progressive program leadership in health care or another complex regulated environment.
• Experience leading and presenting cross-organizational transformation, executive governance, integrated program controls,

vendor oversight, financial planning and transition-to-operations.
• HIS/EHR, Epic or clinical-transformation experience is strongly preferred; PMP, PgMP, Prosci Change Management, Lean or ITIL is an asset.
• Strong familiarity with MS 365 applications
• Enterprise integrator who can connect strategy to execution, build scalable delivery capability, communicate clearly with executives and influence across organizational boundaries.
