Position: Manager of Professional Practice & Clinical Transformation, Epic Connect THP/HH Status: Temporary Full-Time (24 months) Number of Vacancies: 1 Department: Epic Connect Role Level: $112,125.00 – $168,187.00 (salary band placement commensurate with experience) Reports To: Associate Vice-President, Epic Connect THP/HH Posted: Tuesday, October 20, 2026 – Tuesday, November 3, 2026 Strategic Context Trillium Health Partners (THP) and Halton Healthcare (HH) are formally exploring whether HH should join THP’s Epic environment through Epic Connect. The due diligence and planning phase is focused on the assessment and planning to validate the opportunity, confirm requirements, and determine the recommended direction. It does not represent a final implementation decision or commitment. Together, HH and THP aim to create a more connected, accessible and digitally enabled regional system of care that improves patient outcomes, strengthens team and learner experience, and makes the best use of public resources for the communities they serve. The potential partnership is a clinical transformation enabled by digital, operational, and governance alignment. The due diligence and planning phase will assess clinical workflow and THP foundation fit, technical and infrastructure requirements, scope, total cost of ownership, resourcing, governance, organizational readiness, stakeholder engagement and the future operating model. Findings will inform a proposed Epic Collaboration Agreement and implementation recommendations for approval by the appropriate governance bodies. The Position The Manager of Professional Practice & Clinical Transformation will lead THP’s contribution to clinical workflow and foundation validation during the due diligence and planning phase. This role will help HH clinical leaders assess current state of HH clinical practice and standards, understand THP’s Epic foundation, assess fit, identify clinically necessary variation in practice, and define the clinical readiness conditions for a potential shared environment. HH remains accountable for its clinical transformation, implementation, clinical adoption, organizational change management, communications, and risk management. The Manager of Professional Practice & Clinical Transformation will strengthen HH’s leadership through structured collaboration, evidence-informed methods, and practical recommendations that support its accountability. If the organizations proceed following due diligence, the role will transition into the implementation team, contributing to detailed planning, clinical workflow alignment, readiness, go-live, and post-go-live stabilization with Halton Healthcare. Key Accountabilities
• Apply the approved patient, quality management and safety principles and frameworks to ensure clinical outcomes, quality, safety, access and experience all guide decision-making.
• Lead a structured current-state assessment of HH clinical practice, models of care, professional roles and accountabilities, workflows, standards, and supporting policies, engaging clinical and operational leaders to identify strengths, variation, risks and opportunities.
• Translate current-state findings into future-state clinical practice implications, workflow, system, documentation, reporting requirements and decisions.
• Lead structured review of THP’s Epic foundation and workflows with HH clinical leadership to validate clinical scope and identify strengths, gaps, risks and opportunities for alignment.
• Test the shared assumption that 80–90% of THP’s existing Epic foundation can be leveraged,

applying standardization with purpose and identifying variation where justified by clinical need, evidence, legislation, regulatory or professional requirements, quality, safety, access, outcomes or mutual benefit.
• Facilitate interprofessional and cross-functional engagement, including physicians, nursing, allied health, Professional Practice, Clinical Informatics, operations, quality and patient safety, to validate workflows, identify risks, resolve practice requirements and shape recommendations.
• Define the clinical governance, decision rights, escalation pathways and documentation requirements for shared workflows and future change requests.
• Identify and assess patient-safety, quality, workflow, workload, regulatory, scope-of-practice, human factors, usability and adoption risks and integrate them into program governance and decision-making.
• Advise on HH implementation-readiness requirements, including clinical leadership, engagement, practice change impacts, education, training, adoption and support, while preserving HH accountability.
• Define clinical benefits, baseline measures and an evaluation approach to assess readiness, adoption, workflow performance, safety, experience, and shared outcomes. What Success Looks Like
• HH clinical leaders understand THP’s Epic foundation, the implications of adoption and the practice changes required to support a shared workplace.
• Current-state clinical practice, models of care, workflows, strengths, variation, gaps and risks are documented and validated with HH leadership.
• Fit, gaps, and clinically, regulatorily or operationally justified variation are documented, with mutually beneficial enhancements identified.
• Clinical governance, decision rights, safety oversight, readiness, implementation-resource and adoption requirements are clearly defined.
• Professional practice, regulatory, clinical standards, and patient-safety requirements are appropriately reflected in recommended workflows and HIS requirements.
• The recommendation is grounded in evidence, patient outcomes, quality, safety, access, experience and clinician usability. Qualifications and Leadership Profile
• Registration in good standing with an Ontario regulated health profession, with a clinical background in an acute-care hospital environment, is required.
• A master’s degree in a health discipline, nursing, quality, informatics, health administration or related field is required.
• 5–7 years of clinical experience in an acute-care hospital environment required.
• 3
- 5 years of progressive clinical leadership in a complex hospital or health-system environment preferred.
• Strong understanding of models of care, interprofessional practice and the relationships among clinical practice, workflow, workforce and technology is required.
• Strong knowledge of healthcare legislation, professional regulation, scope of practice, and the governance, standards and accountability requirements of regulated health professionals.
• Strong knowledge of applicable provincial, regional and specialty clinical standards and guidelines (e.g., critical care, cancer, emergency,

cardiac, stroke, renal, rehabilitation, trauma and surgical care).
• Demonstrated leadership in Professional Practice and/or complex clinical transformation, with experience partnering across clinical operations, quality and Clinical Informatics.
• Experience supporting large-scale HIS/EHR implementation or optimization, including the application of human-factors and usability principles to translate clinical practice requirements into safe, effective and user-centered digital workflows; Epic experience is strongly preferred.
• Clinically credible systems thinker with the ability to apply evidence and data to assess variation, evaluate impact, inform complex practice and standardization decisions, and translate strategy into safe practice change.
