As a Social Worker, you will collaborate with the Director of Care and Business Manager to facilitate incoming resident transitions.

Your role includes supporting families during move-ins, assessing individual needs, and participating in the Continuous Quality Improvement (CQI) program to enhance healthcare services. Key Responsibilities:
Support residents and families during the move-in process
Assess residents’ financial and health-related concerns
Coordinate with residents' Power of Attorney as needed
Participate in policy evaluation and CQI activities
Prepare reports for residence management and corporate office Requirements:
Bachelor’s or Master’s Degree in social work
Minimum one year of relevant healthcare experience
Preferred experience in long-term care settings
Commitment to resident-centered care principles
