Renfrew Victoria Hospital is launching ALIGN, a program focused on secure hospital-to-home transitions, with a full time Registered Practical Nurse (RPN) role. The RPN will work across inpatient and community settings, providing assessments, medications, education, and discharge planning.

ALIGN emphasizes coordinated, patient-centred care, collaboration with multidisciplinary teams, and support for complex needs to reduce delays and improve outcomes for patients and families.
