This position requires experience in palliative care and solid communication skills. You’ll be responsible for patient assessments, individualized care plans, and direct hands-on care to enhance comfort and dignity during the final stages of life. Key Responsibilities:
Conduct ongoing comprehensive patient assessments
Develop and evaluate personalized care plans




Provide medication administration and advanced wound care
Manage palliative care emergencies proactively
Educate patients and families about end-of-life processes Requirements:
Minimum three years’ experience in end-of-life care
Registration as an RPN with the College of Nurses
Excellent communication and critical thinking skills
Demonstrated teaching and training abilities
