Service Coordination for Persons With Special Needs is seeking a Case Manager for Children’s Services to coordinate supports for eligible children with developmental disabilities or autism. You will work directly with families to access community resources and implement person-centered plans, while maintaining case files and collaborating with partners.

The role requires 3+ years in developmental services, a BSW/DSW/SSW diploma, and robust communication skills.
