In this role, you will work under the guidance of the Home & Community Care Manager to deliver quality care in community settings. Your independence and initiative will be key in coordinating care, supporting health workers, and collaborating with a multi-disciplinary team.

Key Responsibilities:
• Screen, plan, and coordinate client care
• Support and direct community health workers
• Perform intake and health assessments
• Provide personal care and health teaching
• Develop and implement care plans

Requirements:
• Valid LPN license in B.C.
• Ability to work independently and collaboratively
• Solid communication and assessment skills
• Experience in community health settings preferred
