Durham Community Health Centre is seeking a Peer Support Facilitator for the 2SLGBTQI Health & Wellness program in Pickering, Ontario. This temporary full time role reports to the Manager, Integrated Care and Experience and supports clients through peer-based care, navigation, and advocacy.

The position emphasizes lived experience, confidentiality, and collaboration within an interprofessional team to foster an inclusive, welcoming environment for gender-diverse individuals seeking care.
