As a Habilitation Expert, you will be instrumental in creating and executing individualized habilitation plans while supervising a dedicated team of Habilitation Assistants. This role invites you to actively engage in person-centered planning and develop sticky connections with program participants, families, and service coordinators. Your leadership will guide team meetings and ensure the effective tracking of participant progress.

Key Responsibilities
Create individualized goals reflecting participant strengths
Oversee implementation of treatment in various settings




Mentor and manage the Habilitation Assistant team
Act as a liaison for participant circles of support
Document progress and service provision in detail

Requirements:
Bachelor's degree in Human Services or similar
Minimum one year of related experience required
Solid communication and organizational abilities
Skills in operating adaptive and supportive devices
Valid NYS driver's license preferred
