The Community Health Centres of Northumberland is seeking a Registered Nurse to join the Diabetes Education Team. The role is permanent, full time (35 hours/week) and includes advantages with HOOPP enrollment.

You will deliver diabetes education, clinical support, and practical guidance to adults living with diabetes across our communities in a person-centred approach. You’ll collaborate with dietitians and primary care providers, participate in program development and evaluation, and travel to
