As a full time temporary Discharge Planner, you will utilize your nursing expertise to coordinate discharge for patients at both Huntsville District Memorial Hospital and South Muskoka Memorial Hospital. This role demands collaboration with interdisciplinary teams and requires managing community services, referrals, and transportation. Additionally, you will address patient needs and ensure efficient discharge procedures.

Key Responsibilities:
• Coordinate discharge planning from admission through discharge
• Complete assessments to identify patient barriers




• Participate in interdisciplinary rounds and discussions
• Arrange community services and follow-up care for patients
• Support patients at risk of Alternate Level of Care (ALC)

Requirements:
• Current registration with the College of Nurses of Ontario
• 3 years of acute care hospital nursing experience
• Basic Life Support (BLS) certification
• Knowledge of hospital protocols and procedures
• Excellent competency with Microsoft Office Suite
