The Health Centre in Kitchener-Waterloo is recruiting a Community Health Worker for a full time fixed-term 6-month contract. The role focuses on building programming for priority populations and social networks, with a view to addressing social determinants of health within the community.
You will collaborate with CHC colleagues, facilitate groups, support referrals, and contribute to program calendars while maintaining client confidentiality and accuracy in health records.
