As an integral member of the CoAct team, you will collaborate with multidisciplinary professionals to assist elderly clients in achieving their care goals. This role requires skills in personal care, effective communication, and understanding of resident health needs. You will maintain a clean and secure environment while monitoring residents and reporting progress to nurses effectively.

Key Responsibilities:
• Provide personal assistance supporting daily living activities
• Collaborate in resident care planning with families and staff




• Monitor and report residents' well-being to Registered Nurses
• Maintain cleanliness and safety in patient environments
• Document resident care accurately and timely

Requirements:
• Active registration with CLHA as a Health Care Aide
• Completion of Health Care Aide Certificate within 18 months
• Current BLS and Supportive Pathways Certification
• Minimum of one year as a Health Care Aide preferred
• Experience with dementia patients is an asset
