This full time, contract role as a Registered Nurse Case Manager lasts until September 2027. You will collaborate on a multidisciplinary team to support individuals emerging from homelessness, conducting assessments and developing care plans tailored to their needs. Your proactive approach will ensure clients receive holistic care and community resource connections.

Key Responsibilities:
• Conduct face-to-face assessments with clients in varied locations
• Develop coordinated care plans utilizing OCAN assessments
• Provide community-based supports addressing education and crisis intervention




• Administer medications in partnership with the psychiatrist
• Facilitate referrals to community services for comprehensive support

Requirements:
• Bachelor’s Degree in Nursing; CNO registered
• Minimum 2 years nursing experience, preferably in community outreach
• Experience with concurrent disorders and unstable housing
• Commitment to Housing First principles and mental health knowledge
• Proficient in interpersonal and organizational skills
