Key Responsibilities:
• Manage daily clinical priorities and referrals
• Collaborate with the Nutrition Manager and care teams




• Review Resident charts and assess nutrition risks
• Document assessment findings and follow care plans
• Engage with Residents during meal times for nutrition support

Requirements:
• Registered with the College of Dietitians of Ontario
• Robust written and verbal communication skills
• Excellent organizational abilities
• Problem-solving and critical thinking skills
• Proficiency with EMR systems
