As a GEM Nurse, you will participate in comprehensive geriatric assessments alongside patients, families, and community partners. Your background should include at least two years of experience in elder care and a robust ability to implement and manage programs tailored for older adults, ensuring advocacy for best practices in their care.
Key Responsibilities:
• Conduct comprehensive geriatric assessments in the ED
• Collaborate with multidisciplinary team members and community resources
• Provide health education to individuals and groups




• Demonstrate leadership in program management for elderly care
• Support a culture of safety in patient care
Requirements:
• Current Ontario Nursing Registration Certificate
• BLS Certification required
• Provincial Geriatrics Leadership onboarding completion
• Minimum two years of elder care experience
• Bilingualism in English/French preferred
