Trillium Support Services - Community Support Worker - Greater Toronto Region

Hiring experienced and professionally trained Support Workers (CYW, DSW, CSW, SSW, Behavioural Sciences) to provide high-quality care and behavioural support to individuals with complex needs, including developmental disabilities and autism.

Position Overview:

As a Community Worker with Trillium Support Services, you will provide person-centred support in family homes, community, and residential settings. The focus of this role is supporting daily activities, behaviour management, skill building, promoting independence, and ensuring a safe, respectful, and consistent environment.

What You’ll Do

- Provide consistent individualized support in a respectful, person-centred manner
- Implement an individualized Behaviour Support Plan
- Support emotional regulation, communication, and daily living skills
- Personal care as required
- Engage in meaningful daily activities that build independence and confidence
- Accurately collect and document behavioural data
- Use positive behaviour support strategies to reduce challenging behaviours
- Maintain a calm, safe, and predictable environment
- Work collaboratively with the support and clinical team

Who You Are





- Diploma or Degree in CYW, DSW, CSW, SSW, Behavioural Sciences, or related field
- Minimum 2 years of direct experience supporting individuals with complex needs, developmental disabilities, autism, and behavioural support
- Knowledge of positive behaviour support (ABA experience an asset)
- Strong documentation and communication skills
- Reliable, professional, and committed to team-based care
- First Aid & CPR required
- Crisis Management Training (SMG, CPI) required
- Vulnerable Police Check required/Broad Record Check (within 6 months)
- Valid drivers license and reliable vehicle
