Ontario Health atHome seeks a Care Coordinator (RN, PT, OT or SW) for a full-time role starting immediately in North Bay or Kirkland Lake. You will assess patient needs, determine eligibility for services, and connect patients with community resources and care teams.

You will work in a team-oriented, multidisciplinary setting to support patients and families, ensuring accessible, patient-centered care at home and a strong emphasis on continuity of care.
