CHEO is seeking a Care Coordinator to deliver connected, patient-centred home and school care for children and youth in the community. You will perform assessments, develop and evaluate care plans, and coordinate services after discharge with families and CHEO teams.
The role requires pediatric assessment skills, registration with a regulatory college, and at least two years of experience; bilingual English/French is essential for Ontario. Proficiency with EPIC/CHRIS is expected.
