Ontario Health atHome in Lindsay is seeking a Care Coordinator (RN, physiotherapist, OT, or RSW) to assess patient needs, determine eligibility for services, and coordinate referrals to community resources. The role supports patient-centered care in home and community settings across Ontario.

You will work with patients, families and health practitioners to develop care plans, monitor delivery, and ensure smooth coordination.
